It gathers input about the childâ (TM)s participation at home and in the community; daily routines related to eating, sleeping, dressing and toileting, bathing, traveling, playing, and communicating; and specifics related to making changes in routines or transitions. The Family Report is available as a set of 10 8-page forms.
